The Lake Oswego office is at 5 Centerpointe Dr, Suite 400, Lake Oswego, open Monday to Friday 9 AM to 5 PM. FD-258 fingerprint cards are $35, and a second card at the same visit is $30.

The agency asking for your fingerprints decides the method. If it gives you an ORI number or electronic submission instructions, bring them. If it asks for a printed FD-258 card, we roll your prints on the card and hand it back to you to mail.
