Our Lake Oswego office is in Clackamas County, off Kruse Way near I-5 and I-205. The Lake Oswego office is at 5 Centerpointe Dr, Suite 400, Lake Oswego, open Monday to Friday 9 AM to 5 PM. FD-258 fingerprint cards are $35, and a second card at the same visit is $30.
